Glycopeptides Flashcards
1
Q
What are the glycopeptides?
A
Vancomycin
2
Q
What is the MOA of vancomycin?
A
Binds rapidly and irreversibly to the D-alanyl-D-alanine group on the peptide side-chain of the membrane-bound precursor; glycan chain extension, transpeptidation and cross linking inhibited
3
Q
What is the route of administration for vancomycin?
A
PO, IV
4
Q
What gram positive bacteria is vancomycin effective against?
A
- MRSA
- MSSA
- Enterococcus
- Coagulase negative staphylococci
- Streptococci
5
Q
What gram negative bacteria is vancomycin effective against?
A
NONE!
6
Q
What anaerobic bacteria is vancomycin effective against?
A
- Clostridium
- C. difficile
- NOT b. Fragilis
7
Q
What are the clinical indications for Vancomycin?
A
- MRSA infection: skin, soft tissue, pneumonia, meningitis
- C. difficile infection: severe ot recurrent, PO only
- Coagulase negative staph infections (IV lines, hardware)
- MSSA infection in pts with severe allergy to PCN and Cephalosporins
8
Q
What are the SE for vancomycin?
A
- Hypersensitivity reactions
A. anaphylaxis - “red man syndrome” d/t IV infusion that is too fast
- dose-dependent ototoxicity and nephrotoxicity
- immune-mediated thrombocytopenia
