Primary biliary cholangitis

2) Labs: Include most specific Ix (90%)
1) Pruritis, hepatomegaly, & fatigue
2) Elevated AlkP, ANA (70%), and Antimitochondrial Ab (90%)
Esophageal Spasm: Typical SSx?
Sensation of food getting stuck & retrosternal CP
Paraesophageal hernia: XR finding?
Gas-filled stomach protruding through esophageal hiatus. (This is a type of hiatus hernia.)
Importance of AlkP in Dx of liver disease
Elevated AlkP indicates Cholestatic disease.
e.g., Biliary obstruction.
Elderly patient with changing in bowl habit
1) Ix
1) Colonoscopy is the gold standard in order to rule out CA.
Cf. FOBT is a screening tool that is not appropriate for symptomatic Pt (bowl habit change). Sigmoidoscopy does not examine entire colon.
Amsterdam Criteria
1) Purpose
2) 3-2-1 Rule
1) When to suspect HNPCC (Hereditasry non-polyposus colorectal cancer; Lynch syndrome)
2)
3 family members (1 of whom is 1st degree relative)
2 successive generations
1 individual diagnosed before age 50
