What is the pathophysiology of Gestational diabetes?
The cells become insulin resistant’s which increases the glucose levels.
What are some typical indication of gestational diabetes
- Increase HbA1c
- High polycose or GTT
- LGA baby
- Glucose in the urinalysis
What is the normal range of HbA1c?
0-40 mmol
What is the normal range of a Polycose?
3.0-7.8 mmol
What is the referral process of a GDM insulin dependent?
Transfer of care
What is the referral process of GDM diet control?
Consultation
What are some of the complications that could arise for a GDM wahine?
- LGA, Macrosomia pepi
- SID
- High BP
- Preeclampsia
- Shoulder dystocia
- Increase risk of caesarean section
Definition of GDM
Gestational diabetes mellitus is defined as abnormal glucose tolerance that detected or develops in pregnancy
What are the risk factors of GDM
Overweight/Obesity Excessive weight gain in pregnancy whanau history of diabetes Prev history of GDM Ethnicity Polycystic ovarian syndrome
A wahine with a normal HbA1c BMI no family history of diabetes or GDM which test would you offer her
- 1 Hour polycose test.
Rational there are no increase risk factors to suggest she is more at risk of developing GDM
What is the normal range of a polycose test?
3.0 - 7.8
A wahine with a normal BMI a HbA1c of 40mmoL at booking and a sister who has had GDM which test would you recommend?
- GTT 2Hr test.
Rational: She had 2 risk factors that would increase the likelihood of her having GDM.
What is the normal range of the fasting & the 2hr glucose test?
Fasting < 5.5
2 Hour < 9.0
How much sugar is in the 1 hour test drink?
