What causes tinea infection?
Dermatophyte fungi including trichophyton rubrum
What are clinical features of Tinea capitis?
Scaling of the scalp with pruritis, erythema and crusting
Can be areas of alopecia- can get scarring alopecia without treatment
Can lead to kerion- boggy puss filled crusty mass- urgent referral required
Most common cause- trichophyton tonsurans or Microsporum canis (from animals) will fluoresce green under woods lamp
Treatment
- trichophyton= terbinafine
- microsporum= griseofulvin
- topical ketoconazole shampoo for first 2 weeks
What are clinical features of tinea corporis?
Trichophyton rubrum or verrucosum are main causes
Well defined annular, erythematous lesions with pustules or papules
What are clinical features of tinea pedis?
Interdigital infection is common
White/erythematous cracked and fissued skin which is scaly
What are clinical features of tinea cruris?
In the groin- red brown plaques with uniform scaling
What is the management of tinea?
Mild non extensive disease
1) Topical antifungal = terbinafine or imidazole cream
2) add mild topical steroid (hydrocortisone) if marked inflammation
Severe extensive disease
1) Oral Terbinafine
What are features of pityriasis Versicolor?
Superficial fungal infection of epidermis
Caused by overgrowth Malassezia yeast
What are clinical features of PV?
Multiple oval or confluent patches on trunk
Can be scaly or itching
Can be fawn coloured, pink or white and more apparent in summer
What is the management of PV?
Small area- clotrimazole cream
Extensive area- ketoconazole 2% shampoo OD for 5 days
What are features of erythasma?
Chronic superficial skin infection caused by the overgrowth diphtheroid corynebacterium minutissimum
What are clinical features of erythasma?
Enlarging well circumscribed flat pink-brown patches
Typically on inner thigh, groin or axilla
