What is Flight Safety?

A
  • A conscious attempt to improve procedures and working practices both on the line and in the hangar, in order to meet the desired level of airworthiness. This allows the aircraft to fly safely without endangering personnel or the public.
  • Identification of hazards or risks
  • Method of reporting or investigations of FS occurances and identification to prevent reoccurrences
  • A JUST culture
  • Contributing factors to flight safety incidents include: Fatigue, skill fade, lack of experience, failure to follow procedures FFP, human errors or even a drop of moral or motivation of the tradesman. Mainly HUMAN FACTORS.
  • Flight safety is everyone’s concern.

Flight safety committee meetings

A

They attend a meeting termly to discuss any issues that may have been raised, usually based around flight safety log entries. Meeting includes personnel from different departments and watches. Flight Safety Officer FSO will review the Flight safety log weekly. Any issues needing further input are put before all station FSO at Yeovilton station meeting held termly.

What is a DASOR?

A

Used to report all air safety related occurrences, be it notification of an event which has already happened or identification of a potential air safety hazard. The primary method for submitting a DASOR is through the Air safety information management system (ASIMS). Only available on internal MOD networks and used for reporting and management of air safety occurrences and investigation information.

How do you raise a DASOR?

A
  1. Go onto ASIMS and create a new DASOR
  2. This gets sent to the occurrence managers (SPLOT & AEO)
  3. They decide how to action it.

  • Not to interfere with health and safety equipment

What is COSHH?

A

COSHH sheets are available and held in the logs section, data sheets are held in issue centre which detail the harmful effects, flash point, first aid actions etc of the substance you’re using and the PPE required to counteract the effects.

In the absence of COSHH sheets, what would you do?

A
  • Check for the manufacturers data sheets, readily available on line
  • carry out a risk assessment
  • Assume the worst case scenario and instruct your AET to wear full PPE.
