State the purpose of fault recording and reporting
Ensures airworthiness whilst maintaining a cost effective utilisation of assets through improved reliability and maintenance.
How would a routine fault be recorded?
Through fault coding on GOLDesp
\What is a Narrative Fault Report and when would one be raised?
MF760, raised to provide a comprehensive description of specific faults and circumstances of recovery.
It is used to gather more detailed information to undertake an investigation to isolate the cause and/or explain an unexpected failure on a piece of equipment that warrants further investigation.
When would a MF760 be raised?
- In support of a Serious Fault Signal
- At the discretion of a responsible officer
- If a fault is found in Not In Use equipment
- Faults considered worthy of PT attention by local management
- When instructed to do so by the PT either directly, through an SI(T) or MFRI When
- instructed to do so by a Service Inquiry/ board of enquiry
What is the procedure for raising a MF760?
- Originator/ Squadron completes MF760
- MF760 is forwarded to trade CPO for vetting
- Then input onto RESOLVE via relevant trade manager
- Once completed passed for final staffing and to be authorised by AEO completes technical sections
- Squadron Trade Manager Uploads to Resolve for action by MOSST TA technical Assistant
- Originator to ensure item is quarantined pending investigation, completing MF731 correctly and attaching MF760E (red tag) and a copy of MF760
- PT confirm requirement and initiate investigation, raising a MF761A
- Faulty item is sent for investigation iaw MF761A
- Fault investigated by industry, they compile a MF761A – outstanding actions completed by PT
- MOSST informed of MF761 content
- MF 760 originator is sent a report and completed MF761
What is an MFRI?
Mandatory fault reporting instructions are issued and managed internally by the PT when MOD forms 760 are required on specific items of equipment, promulgated in the Topic 2 N Leaflet 133P.
Details items by part no# and specifies reporting occasions.
What is a Serious Fault Report?
It is a signal raised by the maintainers when they believe they have found a potential severe flight safety risk that could be common on all marks of that aircraft type. It is a quick and direct way of disseminating the information to other squadrons, the PT and force HQ. In response to the receipt of a SFS, the PT will issue a UTI.
When would a SFR be raised?
As a guideline, on discovering a fault that;
- Affects safety of personnel
- Puts the security of airborne comms equipment at risk Has an adverse effect on airworthiness
- Means operating effectiveness or availability is affected
This is however not set in stone, and a maintainer can raise a SFS if he feels it’s justified
Who’s responsibility is it to raise a SFR?
The maintainer in charge of the activity which led to the discovery of the fault.
What is a MF765?
Unsatisfactory Feature Report, relating to documentation/publications. MF765X is for the aircrew publications
When are MF765’s raised?
- On finding factual errors or conflicts in the text/diagrams of the air publications
- When there is considered a better way of undertaking a task than that described in the technical instruction
- When the content or design of a form/document is found to present problems in its use
- When raising a MF765, a suggestion on how to improve the area of MF765 action is to be included.
What are the benefits of raising MF765’s?
Increases accuracy of the technical information available allowing an amendment to be created in CIETP/pub. Can prevent possible damage to aircraft or equipment to maintain airworthiness and saves time and excessive loss of manpower and associated cost.
Whos authorises MF765 on 846 prior to submission and how are they submitted?
Originator forwards 765 to relevant trade manager who establishes that issue not already had a 765 raised against it
Trade manager to staff it on SMR behalf
SMR Authorises all 765’s, delegated by AEO
Trade Manager will then submit on RESOLVE system for action by MOSST/ PT. Trade manager monitors progress of 765

MOSST Query Request Form Merlin On Site Support Team
Can use a Routine/urgent/immediate status
Once completed it goes to (MOSST WO) who liases with component authority for a decision to the question
Use it as a way to push the risk onto the DA by having an LOAA (Letter of airworthiness authority) person accept the risk from a fault or issue
What queries can MOSST respond to?
- Provide advice to maintain operational capability
- Provide repair advice
- Validate maintenance procedures
- Validate trail installations
