How does an extradural haemorrhage typically occur?
Young pts following minor assault/sporting injury
-blow to side of head causing fracture/tearing of middle meningeal artery
Blood accumulates over mins/hrs
How does an extradural haemorrhage typically present?
Brief duration of unconsciousness/lucid recovery
Progressive hemiparesis/stupor
Ipsilateral dilated pupil –> Bilateral, fixed, dilated pupils
Hemiplegia/resp arrest
What imaging modality is used to assess extradural haemorrhage?
CT
What are the signs of an extradural haemorrhage on CT?
Characteristic lentiform shape
Midline shift
Ventricular compression
What is the appropriate management of extradural haemorrhage?
Urgent neurosurgical referral (Burr hole)
Conservative management if v. minor
