Woman has a UTI with gram negative rods. What is the likely bug?
E. coli (75-95% of community acquired) Staph saprophyticus (20%, more in younger patients)
How do you treat standard, uncomplicated UTI in females?
TMP-SMX
Nitrofurantoin
Fosfomycin
All three of these are considered first line therapy
What do you do if your patient has UTI in an area with high E. coli resistance to TMP-SMX?
Either Nitrofurantoin or Fluoroquinolones (Ciprofolxacin)
What do you do if a patient has 3 or more UTIs in one year in an area with lots of TMP-SMX resistant E. coli?
Prophylactic antibiotics Nitrofurantoin TMP-SMX Ciprofloxacin Not on prophylaxis forever (usually like 6 months)
Why aren’t Fluoroquinolones not considered first line drugs for UTIs?
Because resistance develops rapidly, so we try to keep them in reserve.
Who wouldn’t you want to treat for UTI with TMP-SMX?
Pregnant woman (sulfonamide contraindication)
