Ex testing and stress testing Flashcards
why perform a stress test?
- evaluate cardiopulm symptoms: chest pain, pressure, SOB/DOE, fatigue
- determine disease severity/prognosis
- evaluate effects of surgical/mediccal interventions
- determine dysrhytmia presence/management
- determien functional capacity; screen for exercise partipcation/prescritipn/return to work
goal=ilicit symptom
what is a peak exercise test?
the symptom is limited (e.g. fatigue/SOB/DOE)
what is a max exercise test?
specific conditions must be met
for ex: ECG changes (ST segment elevation/depression); angina, “criteria for max”
give some examples of submax tests
5xSTS
6 MWT, shuttle walk test, step tests, cycle tests
TUG, BERG
what is the criteria for Vo2 max?
HR; BP plateau with increasing workload
- within 5-15 beats of predicted HR max
- RER > 1.15
- blood lactate> 8-10
what is it called when the criteria for Vo2 max is not achieved?
Vo2 peak
when should you stop an exercise test?
-inability to monitor patient
-patient request; fatigue/DOE
-angina
-abnormal ECG:
VT/VF; 2nd/3rd heart blocks; rapid AF/flutter
repolarization abnormalities consistent with ischemia :T wave inversion…S-T segment elevation/depression
-abnormal hemodynamnic responses: decrease SBP>10 with increase workload HTN: resting BP>210/110 SBP should never go down exercising SBP>250 and DBP>115
-HR responses
if > 75%
-pulm responses: SpO2 <88
-excessive DOE or other symptoms
RPE >17 or 7
- dizziness/pallor
- confusion/balance changes
