o Most attributes to alcohol and tobacco use
o Most common worldwide but decreasing in Canada
Etiology/Risk of Adenocarcinomas
o Increase in adenocarcinomas is associated with Barrett esophagus
o Pt with Barrett esophagus are likely to get esophageal CA
Patho of Esophageal CA
- multistep process
- dev of dysplasia
When is progressive dysplasia apparent in esophageal CA? Is it an early or late mnfst?
- apparent first w/ bulky food
- later w/ soft food
- then w/ liquids
- dysplasia is a late mnfst
Manifestations (4)
o Unintentional weight loss
o Anorexia
o Fatigue
o Pain on swallowing
Diagnosis
endoscopic surveillance of persons w/ Barrett esophagus to detect during an earlier stage
At what stage is most amendable to curative surgical resection?
early stage
Treatment
o Depends on tumor stage
o Surgical resection:
- cure when done early stage
- Palliative in late stage
o Radiation
- used as an alternative to sx
o Chemotherapy
- used before sx to decrease the size of tumor
- used with irradiation and sx to increase survival

Can esophageal CA be improved?
yes but POOR
