what is a thyroid nodule?
Thyroid nodules are solid or fluid-filled lumps that form within your thyroid. The great majority of thyroid nodules aren’t serious and don’t cause symptoms.
what are some risk factors for a thyroid nodule?
- iron deficiency
- thyroid adenoma
- thyroid cyst
- thyroiditis
- multinodular goitre
- thyroid cancer
- exposure to radiation
- smoking
- family history
what are some clinical features of a thyroid nodule?
Most thyroid nodules don’t cause signs or symptoms. Occasionally, however, some nodules become so large that they can:
- Be felt.
- Be seen.
- Press on your oesophagus, causing shortness of breath or difficulty swallowing.
what signs on examination may you find for a patient with a thyroid nodule?
Movement of thyroid on swallowing / enlargement or asymmetry / lumps /lymphadenopathy.
In some cases, thyroid nodules can produce additional thyroxine. The extra thyroxine can cause what kind of symptoms?
Unexplained weight loss. Increased perspiration. Tremor. Nervousness. Rapid or irregular heartbeat.
which conditions can present in a similar way to a thyroid nodule?
Goitre. Grave’s disease. Hashimotos thyroiditis. Thyroid cyst. Thyroid adenoma. Colloid nodule. Hypercystic nodule.
what investigations would you do to confirm a diagnosis of a thyroid nodule?
- physical exam
- TFTs to see if producing any hormones
- ultrasound to distinguish cysts from solid nodules
- fine-need aspiration biopsy
what is the treatment for a benign thyroid nodule?
- watch and wait
- levothyroxine (supplying additional thyroid hormone will signal the pituitary to produce less TSH, the hormone that stimulates the growth of thyroid tissue)
- surgery for large nodules
what is the treatment for a thyroid nodule that causes hyperthyroidism?
Radioactive iodine therapy WITH High-dose antithyroid drugs: thiamazole.
Surgery (subtotal thyroidectomy).
what is the treatment for a malignant thyroid nodule?
near-total thyroidectomy
alcohol-ablation
what are some complications of thyroid nodules?
- possible problems swallowing or breathing
- hyperthyroidism
- problems associated with thyroid cancer
