Clinical uses of somatostatin?
- Pituitary excess-acromegaly, TSH-secreting tumor, ACTH-secreting tumor. 2. GI endocrine excess-carcinoid syndrome, ZE syndrome, VIPoma, glucogonoma, insulinoma. Diarrhea. Dec splanchnic blood circ in pt w/ cirrhosis and esoph varices.
Sx of hypothyroidism?
Cold intolerance (dec heat production), weight gain, dec appetitie, hypoactivity, lethargy, fatigue, weakness, constipation, dec reflexes, dry cool skin, coarse britle hair, bradycardia, dyspnea on exertion
Lab findings of hypothyroidism?
Dec free T4 and inc TSH
Causes of hypothyroidism?
- Hashimoto thyroditis. 2. Cretinism. 3. Subacute thyroditis (de Quervain’s). 4. Riedel’s thyroditis.
What is the most common cause of hypothyrodism?
Hashimoto thyroditis
What is Hashimoto’s thyroiditis?
Autoimmune disorder (antithyroid peroxidase antibodies and antithyroglobulin antibodies).
Hashimoto’s is assoc w what HLA?
HLADR5 and HLAB5
Hashimoto’s histology?
Hurthle cells, lymphocytic infiltration w/ germinal centers (thyroid resembles lymph node)
What cancer is Hashimoto’s at risk for?
Follicular B-cell lymphoma
Presentation of Hashimoto’s?
Moderately enlarge, NONTENDER thyroid
What could be an early finding of Hashimoto’s? Why?
Hyperthyroid due to destruction of thyroid follicle releasing thyroid hormones
