Digoxin
class: cardiac glycoside, antidysrhythmic, inotropic agent
INDI: Afib/aflut, refractory svt, chronic CHF Contra-vfib & vtac, 2nd & 3rd heart blocks, use caution w/ pt’s who take digoxin
Dose .25-.5mg IVP
not for kids
diltiazam hydrochloride
Cardizem
Class:Calcium channel blocker, antidysrhythmic, anti hypertx Indi:symptomatic AFIB & AFLUT, SVT & PSVT Contra: BP <90, 2nd & 3rd HB, Sinus Sick syndrome, AMI, WPW
Dose 15-20mg IVP not for kids
dipenhydramine aka benadryl
antihistamine, sedative hypnotic
Indi: allergic symptoms, dystonic reaction extrapyrmaidal symptoms
Contra: acute asthma, nursing mothers, glaucoma
Dose: 25-100 mg/kg IVP or deep IM Peds 1mg/kg
dopamine
sympathomimetic, catecholamine, vasopressor
Indi: cardiogenic shock, distributive shock after fluids, hemodynamic hypotension, symptomatic hypotension, symptomatic brady (2nd line drug)
Contra: hypovolemia, tachydysrhythmias, hyperthermia 400mg into 250ml Saline bag for 1600mcg/ml
Dose: 5-10mcg/kg for increased cardiac output, 10-20 mcg/kg for vasoconstriction
Epinephrine hydrochloride
sympathomimetic
Indi: cardiac arrest, asthma, allergic reaction or anaphylaxis, symptomatic brady(peds), Beta blocker OD (peds)
Contra: hypovolemia, hypertension
Dose: Cardiac 1mg IVP or .01mg/kg peds Asthma/Allergic-.3-.5mg IM (prn 2x 20 min) Peds-.01mg/kg max .5mg single dose (prn 20) Anaphylaxis- .1-.5mg SIVP(1-5 min) Peds-.01mg/kg SIVP max single dose-1mg prn 3-5 min or .01 mg/kg deep thigh max .05 single dose Brady dysthrhythmia peds/infant-.01mg/kg IV or .1mg/kg ET
