How does albumin affect drug distribution?
- low albumin can cause increased free fractions of drugs
- toxic effects are more likely to occur due to albumin being binding/carrier protein for most drugs
What is a the main concern with long term use of PPI’s?
Low B12
Dairy and antibiotics (Abx)
cheltion reaction occurs, do not consume dairy near time of taking antibiotics
- calcium binds to antibiotics
cytochrom p450/furanocoumarin
furanocoumarin inhibits the enzyme cytochrome p450 which is used in liver to break down drugs
- can lead to toxis effects of drugs at lower amounts
- found in grapefruit and other citrus fruits
Long term issues of corticosteroids
Calcium and Vit D levels
Potassium sparing/depleting diuretics
- Lasix: monitor K+ levels (some Mg, Na and phosphorus depletion as well)
What is our education for coumadin/warfarin?
- supplement with vitamin K
- blood thinner
How should we adjust a diet intake for someone of MAOI?
avoid foods high in tyramine
- aged meats
- aged cheese
- soy
things to watch with long term metformin use
- monitor B12
- not compatible with cythroid (hypothyroid medication); makes both medications inactive when taken together
Antibiotic associated diarhhea (AAD) recs
- fluids
- bland foods
- probiotics/prebiotics
- spread out antibiotic and calcium intake (binds to antibiotics)
