What is the minimum developmental age to diagnose DMDD?
six years old. must be present before 10
What is the main symptom identified in DMDD?
Irritability and frequent temper outbursts
Manic Episode
one week or longer of elevated or irritable mood and increased energy with three or more:
inflated self esteem
decreased need for sleep
more talkative
racing thoughts
distractible
etc
Major Depressive
Episode
2 week or longer period of 5 or more
depressed mood for most of the day
diminished pleasure in activities
weight loss or gain
insomnia or hypersomnia
etc.
A manic episode automatically bumps an individual into what
bipolar diagnosis?
Bipolar 1
Bipolar I Disorder
one lifetime manic episode required
(depression is common but not required)
mania is severe to cause impairment in social or occupational functioning OR require hospitalization
Bipolar II Disorder
necessary: one hypomanic episode, one depressive episode, NO manic episode
Cyclothymic Disorder
for 2+ yrs (1 for kids), numerous periods of hypomanic and depressive episodes that do not meet full criteria
never completely without symptoms more than 2 consecutive months
Why is it important to consider developmental context when diagnosing depressive disorders?
You need to consider what is and is not developmentally appropriate for the child, need to understand that the diagnosing criteria are different for children and adults, and need to probe for irritable mood in questioning, as depressive disorders can often present as such in children
What depressive symptoms are more likely to be reported by parents/clients when coming in for an evaluation
or therapy, cognitive or somatic? Why is this?
Somatic symptoms because they are more likely to catch the kids/parents attention first
What differentiates a manic episode from a hypomanic episode?
hypomania is less severe and makes less of an impact on social/work areas. it also is shorter than mania
What are the DSM criteria for DMDD
A: severe temper outbursts out of proportion to situation
B: outbursts inconsistent with developmental level
C: occur 3 or more times a week
D: irritability or angry mood b/t outbursts most of the day
E: 12 or more months without 3 or more months w/out symptoms
F: present in at least 2 settings
Why is Pediatric Bipolar Disorder controversial in the field of psychology?
symptoms overlap with many other childhood disorders and it is hard for the symptoms to be sever enough to be diagnosable in children. they have less free will to present as manic
