  1. put these patients on a cardiac monitor because initial K will be high and then after giving insulin it will cause a shift to possible hypokalemia
  2. rapid isotonic fluid (NS of 20 ml/kg) replacement

what should the nurse know when giving rapid isotonic fluid replacement?

A
  1. watch for fluid volume excess and cerebral edema
  2. once glucose levels approach 250, add glucose (D5 1/2 NS +20 KCl) to IV fluids to maintain 120-240 glucose level
  3. give IV insulin through infusion of 0.1 unit/kg/hr
