aetiology of appendicitis?
unknown, obstruction of lumen, bacterial/viral
pathology of appendicitis?
inflammation of appendix causes greater omentum to stick to appendix and small bowel creating a large inflamed mass
risk factors for appendicitis
age, immunosuppression, diabetes, absence of omentum
pathogenesis of appendicitis?
obstruction of lumen > mucosal inflammation & lymphoid hyperplasia > build up of mucus and exudate > venous obstruction > ischaemia > perforation
symptoms of appendicitis?
central abdominal pain that shifts to right iliac fossa, sore to cough, anorexia, flushed
acute presentation of appendicitis may look like…
vomit, pain, RIF tenderness, inc WCC
signs of appendicitis?
mild pyrexia, tachy, localised pain, guarding, Rosving’s, Psoas, obsturator
what is the localised pain called with appendicitis?
McBurney’s point
what is Rosving’s sign?
pressing on left exacerbated pain in right
what is Psoas sign?
keeps right hip flexed
what is Obsturator sign?
flexing hip and internal rotation causes pain
ix for appendicitis
bloods (CRP-c reactive protein & WCC)*, USS, CXR, urinalysis
what ix would you do with acute presentation of appendicitis?
lilopsoas test
what score is applied to appendicitis to assess severity?
MANTRELS Migration of pain to RIF Anorexia Nausea Tenderness Rebound pain Elevated temp & pulse Leukocytosis Shift of WCC to left
