What is the purpose of chief cells ?
secrete pepsinogen
- pepsinogen becomes pepsin when exposed to acid
- pepsin breaks down proteins
What is the purpose of mucous cells ?
provides protective mucous coat
- protect against self-digestion by HCl and digestive enzymes
What is the purpose of parietal cells ?
produce and secrete HCl
- primary site of action for many of the drugs used to treat acid-related disorders
What is the purpose of hydrochloric acid ?
- maintain stomach pH of 1 to 4
What is GERD ?
gastroesophageal reflux disease
- digestive disorder due to weak cardia muscle of the stomach which leads to acid reflux from the stomach to the esophagus causing heartburn
What is PUD ?
peptic ulcer disease
- too much acid being formed or not enough bicarb or mucous to protect the stomach which will cause stomach ulcers
What are major aggressive factors of PUD ?
- H. pylori
- NSAIDs
- gastric acid
- pepsin
How do you tell the difference between PUD and GERD ?
- PUD: pain usually won’t occur until 2 hrs after eating
- GERD: hurts to eat almost immediately after eating
Which med is our first line of defense for acid related diseases ?
antacids
- will start here and then move onto the next ones
What is the purpose of Antacids ?
use of basic compounds used to neutralize stomach acid
- want to raise the pH of the stomach
Which types of antacids will we use with renal failure pt’s ?
aluminum antacids
Which types of meds won’t stop action of Proton Pump Inhibitors (PPI) ?
- H2 blockers
- antihistamines
What is nausea ?
unpleasant feeling that often precedes vomiting
What is emesis (vomiting) ?
forcible emptying of gastric and occasionally, intestinal contents
What is the worst outcome of constipation ?
toxic mega colon
