Dequervains Tenosynovitis Flashcards
1
Q
ESSENCE
A
Swelling and inflammation of the tendon sheaths in the wrist
Type of repetative strain injury
2
Q
What tendons are usually affected
A
- Abductor pollicis longus tendon
- Extensor pollicis brevis tendon
3
Q
AETIOLOGY
Pathophysiology
A
- Tendon sheath surrounds tendons, formed by connective tissue (synovial membrane), helping to lubricate and protect movement of tendons
- Extensor retiniculum is fibrous band wrapping around dorsal side of wrist. APL and EPB pass underneath this
- Repetative movement of these tendons result in inflammation and welling of tendon sheaths
4
Q
AETIOLOGY
Risk factors
A
- Age between 5th and 6th decade
- Female
- History of similar condition
- Diabetes
- Pregnancy
5
Q
AETIOLOGY
Main cause
A
Repetative movement

Q
CLINICAL FEATURES
Presentation
A
- Symptoms on radial aspect of wrist
- Pain, often radiating to foreaem
- Aching
- Burning
- Weakness
- Numbess
- Tenderness
7
Q
Special test
A
Finkelsteins test
8
Q
What is Finkelsteins test
A
- Patient rests forearm on surface in neutral position with wrist hanging off and unsupported
- Examiner holds thumb and passively flexes it into palm, causing wrist to adduct (ulnar deviation), putting strain on APL and EPB tendons
- If this causes pain on radial aspect wrist then positive
