What is the difference between strain and sprain
A
Strain - injury to muscle or musculotendinous junction
Sprain - injury to ligament

AETIOLOGY
Most common cause
A
Short fall injuries causing ankle to roll (inversion) causing lateral injury

Is lateral or medial injury more common
A
Lateral injury, eversion injury less likely

ANATOMY
Ligaments of ankle
A
- Talo-fibular ligaments (anterior and posterior) on lateral side
- Calcaneo-fibular ligament on lateral side
- Deltoid ligament on medial side

A
Anterior talo-fibular ligament

CLINICAL FEATURES
Presentation
A
- Describe twisting episode where they invert their ankle
- Pain
- Tenderness
- Swelling
- Sometimes able to weight bear, differentiates from fracture

INVESTIGATIONS
First choice
A
- X-ray to eliminate fracture
- Done if tenderness on posterior malleolus or inability to weight bear

MANAGEMENT
General principles
A
- Initial treatment with RICE
- Analgesia - NSAID
- Physiotherapy
- Surgery rarely indicated
