what is cushing syndrome and cushing disease
A
syndrome - excessive levels of glucocorticoids
disease - results from pit. cushing syndrome

4 causes of cushings
A
- iatrogenic - prednisone
- ACTH secreting tumor of pit
- adrenal adenoma and carcinoma
- ectopic ACTH production
- most often small cell of lung

8 Sx of cushing
A
- appearance changes
- obesity, moon face, striae - HTN
- decreased glucose tolerance - diabetes
- hypogonadism
- masculinization in females
- MSK muscle wasting and weakness
- psych disturbances
- impaired immune

initial screen for cushing
A
dexa supression test
- give and night and measure cort in AM
if 5 has - order high dose test
can also use 24hr free cort test

next Dx steps
A
- measure ACTH to see where from
- if low - adrenal - high dose dexamethasone
- cushing disease will have reduced cort
- if ACTH levels still high - ectopic ACTH - CRH
- if ACTH response then cushing disease
- if no response, then ectopic or adrenal - image appropriate area

A
rare tumors that produce catecholamines

A
- HTN
- severe pounding HA
- severe sweating
- palp
- anxiety
- feeling doom

A
- urine screen for breakdown porducts
- metanephrine - urine/serum epi and norepi
- imaging

Tx for pheo
A
- surgery
