what are the ategories under the overdose CPGS?
- opioids
- TCA antidepressants
- Sedatives
- Psychostimulants
How do you manage a heroin overdose?
Assist and maintain airway
- Naloxone 1.6mg - 2mg
If adequate response
- consider transport or treat and refer
If inadequate response:
- Transport
COnsider SGA
How to you treat other opiod overdose?
Assist and maintain airway
- Naloxone 100mcg IV
- > repeat Naloxone 100mcg IV @ 2 mins (Max 2mg) until pt is self-ventilating
IF NO IV ACCESS
Naloxone 400mcg IM single dose
What should be actioned early with Hyperthermic psychostimulant OD?
Early sedation to regulate temperature
How do you manage sedative agent OD?
Airway mgmt
- manage agression as per agitation cpg
- manage perfusion as per inadequate perfusion cpg
How do you manage psychostumulant OD?
Reduce stimuli
- Mx seizures
- Mx cardiac chest pain
- Mx temperature
- Mx agitation
what are the signs and symptoms of TCA overdose?
Mild to moderate OD:
- Drowsiness, confusion
- tachycardia
- slurred speech
- hyperreflexia
- ataxia
- mild hypertension
- dry mucous membranes
- respiratory depression
Severe toxicity:
- coma
- respiratory depression
- hypoventilation
- PVCs
- SVT
- VT
- Hypotension
- Seizures
- ECG changes
What can TCA overdose lead to?
Aspiration
Hyperthermia
Rhabdomyolysis
APO
What are the signs of TCA toxicity?
QRS >0.12 sec
Hypotension
Ventricular arryhtmias
How do you treat TCA toxicity?
MICA
sodium bicarbonate 8.4% 100ml IV @3mins
What PAED management is provided for Opiod OD?
Naloxone 10mcg/kg IM
(Max 400 mcg)
Rpt after 10 mins if required
