what is the median return to normal conscious state after dextrose administration
5 - 15 mins.
When considering additional doses, reassess conscious state and BGL
what do you consider if BGL is back to normal but pt still unconscious?
sepsis, head injury, trauma
What do you do if the patients BGL is 4-11?
consider
- stroke
- seizure
- hypovolaemia
What do you do if the BGL is <4 but pt responds to commands?
Glucose 15g oral
If inadequate response after 15mins: - consider repeat glucose 15g oral (max 30mg) - Dextrose IV or - Glucagon IM
What do you do if the BGL is <4 but pt not respomding
IV cannula Large vein
- confirm patency
- > dextrose 10% 15g (150ml) IV
- > > NS 10ml flush
If GCS or BGL not returned to normal after 5-10 mins:
- Dextrose 10% 10g (100ml) titrated to effect
If unable to insert IV:
- Glucagon 1 IU IM
What is the Paediatric treatment for BGL <4 but responding to commands?
Glucose 15g oral
- repeat 15 min max of 30mg
- Glucagon IM
What is the Paediatric treatment for BGL <4 but NOT responding to commands?
<25 kg = Glucagon 0.5 IU (0.5mL) IM
> 25kg = Glucagon 1 IU (1mL) IM
