CRPS Definition
A
- Pain disproportionate to an injury that is either:
1. Sympathetically maintained
OR
2. Independent of the sympathetic nervous system (traditionally called “reflex sympathetic dystrophy) - CRPS is the primary and most severe complication of hand and distal radius fractures

Type I CRPS
A
- Develops after a noxious event

Type II CRPS
A
- Develops after a nerve injury

Symptoms of CRPS
A
- Allodynia (sensation misinterpreted as pain)
- Hyperalgesia (increased response to painful stimuli)
- Hyperpathia (exaggerated, sometimes delayed response to sensory stimuli)
- Edema
- Contractures
- Bluish or red, shiny skin
- Abnormal sweating and hair growth
- Muscle spasms
- Decreased strength
- Low tolerance for activity

OT Interventions for CRPS
A
- Gentle, PAIN-FREE AROM for short periods (NO PROM and NO painful treatment)
- Stress loading (e.g., scrubbing the floor or carrying a weighted handbag) and functional activities that promote weight bearing
- Pain control techniques (e.g., TENS, splinting)
- Edema control techniques (e.g., elevation, massage, AROM, contrast baths, compression)
- Desensitization techniques, fluidotherapy
- Blocked exercises, tendon gliding
- Joint protection, energy conservation
