When a company officer becomes aware of a facility that does not require the response of a CFRD Company because they have trained personnel, oxygen, and defibrillation
equipment available, the officer can request a “CFR Hold” (CFRH) for that facility.
Therefore, for segment one type calls classified
as cardiac arrest or choking, CFRD units will respond to facilities listed and approved as CFRH in CIDS. For all segment two and three requests, the CFRH will be in effect and
no CFRD unit will be dispatched
In order to remove the facility from the response matrix, the company officer shall ensure
that the facility meets all the following criteria:
On duty personnel are trained in CPR.
* Trained personnel are able to provide oxygen to the patient.
* On duty personnel are trained in the use of a defibrillator, and all the necessary equipment is available to the trained personnel.
* All equipment and appropriately trained personnel are available during all hours of operation. To qualify for removal, the facility shall be operational on a 24/7 basis.
Note: In addition to the above requirements, the facility must be “CFR Hold Eligible” on all floors of the building’s address. The following are NOT eligible for removal from
the response matrix:
*Facilities that do not operate 24/7
*Facilities which are not “CFR Hold Eligible” on ALL floors

3rd Battalion Chief shall be designated as a Sector/Group Supervisor. (2nd Alarm Chief)
4th Battalion Chief shall be designated the Resource Unit Leader. (RUL)
5th Battalion Chief shall be designated the Safety Officer.

A minimum of three Engines, one Ladder, and a Battalion Chief shall be dispatched to any alarm received from a single source (ERS or telephone) reporting an incident requiring a structural response
-Note: Any alarm received from a single source (ERS or telephone) reporting smoke and/or fire in a structure shall receive a minimum response of three Engines, two Ladders and a Battalion Chief.
-If, prior to the arrival of Fire Department units, an alarm is received from more than one source reporting an incident requiring a structural response, the dispatcher shall dispatch a total of three Engines, two Ladders, and a Battalion Chief to that location.
-However, if Fire Department units are already on the scene and a second source is received, the dispatcher will notify the responding Battalion Chief and attempt to contact a unit on the scene. The box will not be filled out by the dispatcher unless
requested to do so by the responding Battalion Chief or the on-scene Incident Commander.
FALLBACK Triggers
Conditions that can trigger the implementation of FALLBACK include but are
not limited to:
A. Sustained Alarm Rate of 140 incidents per hour, for longer than twenty
minutes (not associated with a single incident).
B. Sustained Availability of Engine or Ladder Companies below 50%, for longer
than twenty minutes.
C. A 2nd Alarm or greater transmitted in the borough of Staten Island.
D. Severe weather conditions that are either imminent or occurring, which will
significantly impact activity levels (e.g.: severe thunderstorm, tornado,
blizzard, red-flag warning, etc.).
E. Any 10-66 in conjunction with a 10-60.

- An additional Ladder Company (above the Fast Truck) shall be assigned to a
10-75 when both of the two initially assigned Ladder Companies report
responding with 4 firefighters. - The additional Ladder Company shall be the 3rd Ladder Company, and the 4th
Ladder Company shall be designated as the Fast Truck
When company is understaffed
- When notified that a company is responding understaffed (less than four
firefighters), the dispatcher shall special call another unit (Engine for Engine,
Ladder for Ladder, etc.) to respond in addition to the understaffed unit.
Note: Under this response policy, the total First Alarm response shall not
exceed a total response of:
(4) Engine Companies and (4) Ladder Companies
The following incidents are classified as modified single unit emergencies and are assigned single unit responses. Responding units should operate in the 10-20 mode while
responding to these select call types and signal 10-84 transmitted upon arrival.

The following incidents are classified as modified emergencies requiring the response of
single or multiple units. Units that are assigned 1st due shall respond in the emergency mode. Units that are assigned other than 1st due shall respond in the 10-20 mode.
All units are required to transmit signal 10-84 upon arrival at the reported location.
Response to BARS alarms will be a single unit response – 1st due Engine only
(emergency mode).
Response to Class “E” alarms will be a single unit response – 1st due Engine or Ladder
(emergency mode).
Response to Class “J” alarms between 0700 and 1900 hours will be a
single unit response (emergency mode)
Odors - gas, fumes, etc., other than smoke
Sprinkler activation
Class 3 alarms – automatic, valve, manual, etc.
Electrical emergencies
Manhole emergencies
BARS – 1st due Engine only (emergency mode) -single unit
Class “E” – 1st due Engine or Ladder response (emergency mode)-single unit
Note: When a Battalion Chief or company officer receives additional information
regarding the incident from the dispatcher, he/she may cancel the 10-20 response mode.
The dispatcher shall announce that the 10-20 response is cancelled and that all units
should respond in the emergency mode.
One Engine or one Ladder Company with a Battalion Chief will be assigned to Class “E” alarms citywide, on a 24-hour basis
IF A CLASS 3 IS A CO ALARM –ENGINE AND LADDER
A subsequent alarm of any type from the same address shall call for the remainder of the
full first-alarm assignment. (SECOND SOURCE WILL BE FILLED OUT)
Note: If alarm is determined to be for a Carbon Monoxide Detector activation, the officer
shall notify the dispatcher, then investigate the area of activation regardless of alarm status.
No attempt to reset the alarm shall be made without an investigation of the alarm location.
All procedures listed in Hazardous Materials 4 (Carbon Monoxide) shall be followed.
CLASS J ALARMS….
One Engine or one Ladder Company with a Battalion Chief will be assigned to hotel occupancies (Class “J” alarms) from 0700 hours through 1900 hours citywide. From
1900 to 0700 hours, units will be assigned to Class “J” alarms as per Box assignments
Battalion Chief discretionary response to Class “E” or “J” alarms requires monitoring of the alarm by the Battalion Chief assigned on the Class 3 alarm
When an alarm is received from a Class “E” or “J” alarm system with no additional information and the response is one Engine or one Ladder Company, the Battalion Chief
may either monitor the radio or respond, BASED ON THEIR DISCRETION
-preliminary report not give in reasonable time 5-10 minutes….contact dispatcher
If a subsequent alarm is received while the Battalion Chief is monitoring a Class “E” or
“J” alarm, the Battalion shall NOT be directed to respond unless the dispatcher receives
information of a serious fire and the assigned Battalion is the nearest available Battalion
Chief (i.e. 10-75 or 10-76)
- If the dispatcher decides to assign the Battalion that was monitoring the Class “E” or “J” alarm to another Box, the Engine or Ladder Company investigating the “E” or “J”
alarm shall be notified via Department Radio that no Chief is monitoring the alarm.

The Incident Commander of a Signal 7-5 (Doubtful) structural fire shall not release any units until the fire has been placed Probably Will Hold or Under Control.

Communications Unit
The IC should ensure from the Dispatcher the
response of a Communications Unit to work in conjunction with the Field Communications Unit, if possible, when an auxiliary communications system (ARCS)
is utilized.
Rebreather Unit
Rebreather 1- Staffed 24 hours 7 days a week by 2 Rebreather qualified members trained in the deployment of this long duration SCBA.
Rebreather 1 is equipped with 36 Draeger
BG-4 Extended Duration SCBA (EDSCBA); Rescue Companies, Squad Companies, Haz-Mat Company 1, Haz-Mat Engines and selected SOC Support Ladders are
trained in the use of this EDSCBA.
**he Rebreather unit is quartered at L-20 in lower
Manhattan and RESPONDS on signal 10-75 for an underground subway/railroad and
when special called.
Resources Unit Leader (RESL)
The Resources Unit Leader is responsible for maintaining status information of all
resources allocated to the incident. They are assigned on the transmission of a 10-60 and to all second alarm and greater incidents
not on 10-76 or 10-77 because of FIELD COMM
Field Communications Unit
The Field Communications Unit (FCU) responds as follows
o Second or greater alarms
o 10-76
o 10-77
o Signal 7-5 (doubtful) for fires or emergencies involving below-grade, subway or
railroad facilities.
o 10-60
o Confined Space Incidents
The Incident Commander should return the FCU to service as soon as possible.
If another alarm develops to a proportion where the FCU is needed, the
Incident Commander should evaluate the usefulness of the FCU at the original
incident and release the FCU, if possible

The FDNY IC will be instructed by NYPD to either remain on NYMAC 1 or switch to the assigned NYMAC I/O channel for the incident (NYMAC 2 or 3). The FDNY IC shall notify the appropriate Borough Dispatcher and FDOC of the I/O channel that has been acitvated
When using an I/O channel, the FDNY IC shall use the name of the incident at the beginning of each transmission (e.g. 2nd alarm on Northern Blvd in Queens
When a Communication Engine responds to an incident, or when special called by IC, they will confer with IC and FCU, if already on scene, to determine if a NYMAC channel has already been established prior to their arrival. If NYMAC channel has not been established, the Communication Engine will establish a
NYMAC channel and inform IC once channel has been activated by NYPD.
If channel is established by Communication Engine, they will monitor NYMAC channel. If NYMAC channel is established prior to Communication Engine
arrival, Communication Engine could then assume monitoring of NYMAC channel
NYMAC Channels are found on the APX8000XE HT in the “NYMAC” Zone on channels 2 through 13
UCALL/UTAC are National UHF interoperable radio frequencies used by FDNY Incident Commanders to communicate with public safety agencies. UCALL/UTAC
frequencies are programmed into Zone B on APX8000XE HT radios, DARS radios, and Chief Mobile radios on the apparatus. UTAC channel 42D (Handie-Talkie Zone B- CH 9) is the primary UTAC channel for use in the NYC region.

UTAC repeater channels may be useful at an operation encompassing a vast area where direct HT communications are out of range of one another. In order to activate a UTAC repeater channel, a request is made by the IC via Zone B- Channel 4 (UCALL 40) to the New York City Emergency Management
(NYCEM). NYCEM is responsible for monitoring UCALL 40.
If the IC is
unable to contact NYCEM directly via UCALL 40, a request is to be made via the
Fire Dispatcher to contact NYCEM in order to activate the requested channel.
NYCEM will then activate the necessary UTAC repeater channel. Any requests
outside New York City will go through the agency in command of the incident
