What is the care plan for measuring/recording respirations?
Count the patient’s respirations for one full minute and record your readings

When measuring/recording respirations do you inform the patient you will be counting their respirations?
DO NOT inform the patient; instead tell them you are taking vital signs
Why should you not tell a patient you are measuring/recording respirations?
B/C that will make the patient aware of their breathing and maybe alter their breathing patterns leading to inaccurate readings
What must you do to measure/record respirations?
Place a paper or object on the patient
T/F Hold the wrist as though you are counting their pulse

Count what while measuring/recording respirations?
Count how many times the abdomen rises and falls (in and out = one breath)
How do you finish your skill after measuring/recording respirations?
Ask the patient if they are okay and if you can get them anything else, inform patient you will go wash your hands and document your findings
