metabolic alkalosis with hypochloremia
What is the recommended mode of therapy for severe dehydration?
IV bolus
When assessing hydration status by phone, what are two important finding to inquire about?
child’s level of activity
ability and/or deisre to take fluids by mouth
When is oral rehydration the preferred method for dehydration treatment?
Oral rehydrationis as effective, sfer, and much less costly thatn IV therapy. This should be used when possible
What is the work-up for a suspected pyloric stenosis?
pyloric ultrasound
Upper GI contrast study (if US is unavailable)
Electrolytes to correct hypokalemia, hypochloremia, and alkalosis
What is the differential for recurrent emesis in an infant?
GER
Viral gastroenteritis
Malrotation +/- volvulus
Pyloric stenosis
intussusception
CNS disease
UTI
Can blood be present in GERD?
regurgitation is difficult to distinguish from true vomiting
regurgitation can cause esophagitis –> blood in regurgitation
What are common symptoms of viral gastroenteritis early and late?
early in the course, there is vomiting
later, there is diarrhea
is vomit in malrotation +/- volvulus bilious or non? is blood present in the vomit? Does this shit cause abdominal pain?
bilious
very rarely is blood seen in the vomit. However, it can be seen in the stool
this shit causes pain in the abdomen
What is the classic type of poop in intussusception? What type of mass can be felt in the abdomen? Does this shit cause abdominal pain?
currant jelly stools
sausage-like mass due to the telescoped bowel
this shit hurts
Whta kind of CNS issues can cause emesis?
hydrocephalus, intracranial neoplasm, trauma
can a UTI cause vomiting?
yeah. it can cause cause non-GI vomiting
