clinical jedgment Flashcards
Skills Required For Clinical Judgment
- Critical Thinking
- Clinical Reasoning
Critical thinking involves:
the application of knowledge and experience to identify patient problems and to direct clinical judgments and actions that result in positive patient outcomes
Clinical reasoning is the ability to:
focus and filter clinical data to recognize what is most and least important so that the nurse can identify whether an actual problem is present
Clinical judgment models seek to:
describe the various thought and reasoning activities nurses use when making clinical judgments
Tanner Clinical Judgment Model identifies four key aspects of clinical judgment:
Noticing (being aware of, perceiving)
Interpreting (figuring out, analyzing)
Responding (taking action)
Reflecting (reflection)
Lasater Clinical Judgment Rubric: 3 aspects of Tanner’s Clinical Judgment
Focused observation (Prioritizing data)
Recognizing deviations from expected patterns (Making sense of data)
Information seeking
National Council of State Boards of Nursing (NCSBN) Clinical Judgment Measurement Model include 6 steps:
(1) recognize cues (Identify relevant and important information from different sources e.g., medical history, vital signs)
(2) analyze cues (Organize and link the recognized cues to the client’s clinical presentation.)
(3) prioritize hypotheses (Evaluate and rank hypotheses according to priority)
(4) generate solutions (Identify expected outcomes and use hypotheses to define a set of interventions for the expected outcomes)
(5) take actions (Implement the solution(s) that addresses the highest priorities)
(6) evaluate outcomes (Compare observed outcomes against expected outcomes)
Attributes Required For Successful Clinical Judgment
- A strong knowledge base
- Proficient technical skills
- Early problem recognition
- Effective communication
- Trusting relationships with patients
- Previous experience
- Confidence
- Intuition
- Reflection
Environmental Factors That Influence Clinical Judgment Skills
Task Complexity
Time Pressures
Interruptions
Specialty Area and Autonomy
Professional autonomy
implies self-sufficiency and independence
Strategies To Develop Strong Clinical Judgment Skills
Clinical and Unfolding Case Studies (Active participation in thinking through, answering, and writing clinical and unfolding case studies are excellent ways to improve clinical judgment skills)
Literature Review (accurate clinical judgments cannot be made on topics that are unknown)
Clinical Application of Knowledge and Simulation (Clinical decision-making abilities can be improved when student nurses are required to apply new theoretical concepts introduced in the classroom to patient care in the clinical setting)
Concept Mapping (promotes active learning)
Collaborative Group Learning
Reflection
