Selectively inhibits bacterial protein synthesis.
Active against
Gram +
MRSA
Vancomycin-resistant enterococci
Indications
- Pneumonia
- Complicated skin and soft-tissue infections.
Side effects
- DNV
- Headache
- Blood disorders
- Taste disturbances
- Severe optic neuropathy (see CHM advice).
Contraindications and cautions
- Risk of severe optic neuropathy
- SS
- Acute confusional states
- Bipolar depression
- Elderly
- History of seizures
- Uncontrolled hypertension.
Risk of severe optic neuropathy
o Report visual impairment (blurry/ altered vision)
o Monitored regularly if treatment more than 28 days
Serotonin syndrome
Build-up of serotonin. Altered cognitive state. Potential coma
What other drugs can interact to cause serotonin syndrome
o SSRI
o Dopaminergic
o 5-HT1 agonist
o TCAs
o Lithium
o MAOIs
Interactions
Interacts with tyramine-rich food
Interacts with other drugs
that can cause serotonin syndrome
Do not give with another MAOI or within 2 weeks of stopping another MAOI
Tyramine rich food interactions
Linezolid is a reversible, non-selective monoamine oxidase inhibitor (MAOI).
Patients should avoid consuming large amounts of tyramine rich foods.

Examples of tyramine rich food
o Mature cheese
o Marmite
o Yeast extract
o Fermented soya bean extract
o Some wines and beers
Monitoring
FBC weekly
Blood disorders - close monitoring in those who:
Receive treatment for more than 10-14 days.
Pre existing myelosuppression.
Receiving drugs that may have adverse effects on Hb, blood counts or platelet
Severe renal impairment
Myelosuppression
If significant myelosuppresion occurs, stop treatment unless it is essential.
