what is chronic rhinosinusitis?
A
an inflammatory disorder of the paranasal sinuses and linings of the nasal passages lasting ≥ 12 weeks

what are the predisposing factors for chronic rhinosinusitis?
A
- atopy: hay, fever, asthma
- nasal obstruction e.g. septal deviation, nasal polyps
- recent local infection e.g. rhinitis, dental extraction
- swimming/diving
- smoking

what are the features of chronic rhinosinusitis?
A
- facial pain: typically frontal pressure pain, worse on bending forward
- nasal discharge: clear if allergic/vasomotor, thicker purulent d/c suggests 2˚ infection
- nasal obstruction: e.g. mouth breathing
- post-nasal drip: may produce chronic cough

how do you manage recurrent / chronic sinusitis?
A
- avoid allergen
- intranasal corticosteroids
- nasal irrigation with saline solution

what are the red flag sx for chronic rhinosinusitis?
A
- unilateral sx
- persistent sx despite compliance with 3 months of rx
- epistaxis
