chest pain due to lung pathology must have:
cough, panting or phlegm
heart disease chest pain due to heart vessel impediment and obstruction commonly occurs:
in the elderly
disease course is long and the pain is severe
lung vacuity/deficiency patterns
usually due to consumption of lung yin.
disease course is several years w/ signs of yin def.
liver disease chest pain
disease conditions follow changes in emotions, decrease or increase with emotional situations
possible LR chest pain
qi depression and repression (qi stagnation)
transformation of fire to invade the lung
distending chest pain that occurs with coughing or more severe when taking a deep breath.
vigorous heat
coughing and panting
coughing think yellow phlegm, or blood streaked
tongue: yellow fur
pulse: rapid
heat evil obstructing the lung
external pattern
chest pain especially severe in the cold cough that causes chest pain coughing white phlegm pale complexion cool limbs lack of thirst clear and long urination
tongue: white fur
pulse: slow
cold evil settling in the lung
usu occurs in autumn and winter
external pattern
chest pain in area below the heart does not like pressure in severe cases, hard fullness and pain from area to small abdomen stiffness of the neck thirst sometimes constipation late afternoon tidal fever
tongue: dry
chest bind pattern
external pattern often due to incorrect treatment of taiyang disease, cold evil transforming into heat and entering the interior or water rheum binding inside the chest and stomach
chest pain that is dull cough with scanty phlegm (very sticky or blood streaked) red cheeks tidal fever or steaming bone heat vexing heat of the 5 hearts insomnia night sweating
tongue: purple red, scanty fur
pulse: thin, rapid
consumption pattern
yin deficiency & TB
piercing pain fixed in position panting cough sometimes spitting blood dark facial complexion dark nails and skin dark purple lips
tongue: dark purple, probably macules
pulse: fine and rough/choppy
blood stasis chest bind
angina, MI
blood stasis obstructing internally
chest pain that is oppressive or piercing and radiates to the shoulder and back, inner aspect of upper arm
maybe pain in stomach duct (below heart)
pain comes and goes
heart palpitations
severe condition: sudden and violent pain lips blue green and reverting cold of the limbs clouding of the spirit
tongue: dark body, possibly stasis macules or pale, fat tender with thick slimy white fur
pulse: fine and rough/choppy or bound and regularly interrupted or string like and slippery
Heart vessel obstructing
aka heart attack
often occurs in middle ages or elderly individuals who work and think excessively (ie think too much about how to get ahead)
chest pain with rib side pain
chest oppression
tendency to sign frequently
distending pain or moving and piercing pain or flashes of burning pain
pain increases or decreases with changes n the emotional circumstances
often emotional depression or repression
tendency to be easily angered
pulse: string like
binding depression of Liver qi
distending chest pain or burning rib side pain cough with blood streaked phlegm or coughing of blood dizziness red eyes tendency to be easily angered constipation reddish urine
pulse: string like and rapid
liver fire invading the lung
other causes of chest pain
spinal misalignment
GERD (common reason to report chest pain)
anxiety
ribs out of place, broken, bruised
pleurisy (inflammation of membrane surrounding the lung)
