What does an anterior tooth look like on a charting sheet?
4 boxes, 1 for each surface:
Labial, lingual, mesial and distal
What does a posterior tooth look like on a charting surface?
5 boxes, 1 for each surface:
Buccal, lingual, mesial, distal and occlusal
How is a sound tooth charted?
No change to the box
How are caries charted?
An unshaded shape in the boxes corresponding to where the caries is
How are restorations charted?
Shade the regions corresponding to where the restorations are
How is a failed/leaking restoration charted?
A circle, with a shaded circle in the middle, in the box where the restoration is
How is a missing tooth charted?
A horizontal line runs through the middle of the box
How is a required extraction charted?
A diagonal line running from the top left to the bottom right of the box
How is an extraction treated on this course of treatment charted?
Two diagonal lines running through the box like an X
How is a root canal filling charted?
RCF is written above the box
How is a metal ceramic crown charted?
MCC is written above the box
How is a composite restoration charted?
COMP is written above the box

AM is written above the box
