Association with an underlying hematologic (most commonly acute myelogenous leukemia) or visceral malignancy (most commonly carcinomas of the genitourinary organs, breast, and gastrointestinal tract), inflammatory disease (Crohn disease and ulcerative colitis), or pregnancy, or preceded by an upper respiratory (streptococcosis) or gastrointestinal (salmonellosis and yersiniosis) infection or vaccination
