Physiological barriers result from some kind of sensory dysfunction on the part of either the sender or the receiver. physiological barriers (such as the inability to hear).
The communication process consists of four basic parts: the sender of the message, the message, the receiver of the message, and feedback.
Psychosocial barriers are usually the result of one’s inaccurate perception of self or others;

SOAP NOTE: Subjective-symptoms verbally given by patient, Objective-HM can actually see, hear, feel, touch, Assessment- follows the obj observation the preliminary diagnosis of patient’s condition, Plan-includes lab or radiological test ordered, medication. and a Followup appointment
