inhibits bacterial wall synthesis by binding to specific PBPs; broad-spectrum bacteriocidal
Indications
infections caused by gram positive and a few gram negative bacteria
contraindications
allergy to any drug in this class
precautions
hypersensitivity to penicillins
significant renal impairment since most is eliminated by kidneys
drug interactions
increased risk of nephrotoxicity with nephrotoxic drugs
additive effect with other antibiotics
increases anticoagulant effect of warfarin
adverse effects
rash, diarrhea, superinfections, pain and phlebitis at IM injection sites; allergic response
serious adverse effects
hypersensitivity reactions; seizures
nursing responsibilities
monitor first 30 minutes of IV therapy monitor intake and output ratios monitor for superinfections report bleeding assess IV sites during infusion inject deep into muscle; rotate injection sites
Teaching
consult before drinking alcohol; report signs of infection; report severe diarrhea; do not treat diarrhea with OTC; report bleeding or bruising; report allergic reactions
