CASE ON PUD Flashcards
open sores that develop on the inside lining the stomach and the upper portion of the small intestine.
PEPTIC ULCER DISEASE (PUD)
characterized by discontinuation in the inner lining of the gastrointestinal (GI) tract because of gastric acid secretion or pepsin.
PEPTIC ULCER DISEASE (PUD)
Common types of peptic ulcer
• gastric ulcers
• duodenal ulcers
located in the stomach
gastric ulcers
found at the beginning of the small intestine (also called the small bowel) known as the duodenum.
duodenal ulcers
Common signs and symptoms of PUD
• Epigastric abdominal pain
• Bloating
• Abdominal fullness
• Nausea and vomiting
• Weight loss/weight gain
• Hematemesis
• Melena
Alarm symptoms requiring prompt management:
•Unintentional weight loss
•Progressive dysphagia
•Overt gastrointestinal bleeding
•Iron deficiency anemia
•Recurrent emesis
•Family history of upper gastrointestinal malignancy
• Helicobacter pylori infection,
• Non-steroidal anti-inflammatory drugs (NSAID) use,
• Stress-Related Mucosal Damage (SRMD)
Etiologies
• Zollinger–Ellison Syndrome (ZES)
• cancer chemotherapy
• smoking
• alcohol drinking
Other etiology
slow blood loss leads to anemia or as severe blood loss that may require hospitalization or a blood transfusion.
Internal bleeding
PUD can perforate wall of stomach or small intestine.
A hole (perforation) in stomach wall
block passage of food through the digestive tract causing px to become full easily, to vomit and to lose weight either through swelling from inflammation
Obstruction
studies have shown that people infected with H.pylori have an increased risk of __________.
Gastric cancer
GOAL OF MEDICATION
• Decrease acid secretion
• Protect the mucosa
• Drug therapy
• H2 receptor antagonist (-tidine)
• PPI (-prazole)
Decrease acid secretion
