Diff between USA & NSTEMI?
A
NSTEMI has elevation of Troponin or CK-MB.
Both lack ST elevations & pathologic Q waves
2
Q
Pain control in pt w/ unstable angina?
A
Nitrates & Morphine
3
Q
What enzymes, in particular, tend to require repletion in a pt w/ unstable angina?
A
K+ & Mg2+
4
Q
Medical mgmt of patient w/ unstable angina?
A
- Admit to floor w/ continuous cardiac monitoring
- Give: 1st line= Aspirin, Clopidogrel, β-blockers, Nitrates, Enoxaparin (LMWH), O2 (if hypoxic)
* Morphine (controversial - may mask Sx)
* GP 2b/3a inhibitors - helpful esp if pt undergoes PTCA or stenting - (SOMETIMES) Cardiac cath/revascularization
- After acute Tx: Aspirin, β-blockers, Nitrates
* Reduce risk factors
5
Q
Variant (Prinzmetal’s) Angina - definitive Dx’tic test?
A
Coronary Angiography w/ Ergonovine
- displays coronary vasospasm when patient is given IV Ergonovine
6
