Bullous Diseases Flashcards
Features of HSV1
Primary-painful vesicles on labial mucosa, secondary-grouped vesicles on an erythematous base
Features of HSV2
Primary-bilateral erosions, inguinal LAD; secondary-grouped erosions, shorter duration
Features of varicella zoster
Teardrop vesicle on erythematous base
Complications of VZV
Pneumonia, ataxia, encephalitis, Reye’s syndrome
Features of HZV
Occurs unilaterally along a dermatome, papules/plaques => blisters, disseminated in elderly/immunocompromised
Treatment of HSV and VZV
Acyclovir and valacyclovir
Features of bullous pemphigoid
Tense bullae on urticarial base
Epidemiology of bullous pemphigoid
Elderly 65-75 yo
Pathology of BP
Linear IgG and C3 at DEJ, Ab to BP230 and Bp180 antigens at DEJ
Tx of BP
Topical steroids, systemic steroids, TCN/niacinamide, mycophenolate mofetil
Features of pemphigus vulgaris
Flaccid bullae, Nikolsky +, mucosal involvement
Pathology of PV
Ab against desmoglein 3 and 1, DIF reveals “chicken wire” pattern
Tx of PV
oral steroids, mycophenoalte mofetil, azathioprine, rituximab
Etiology of PV
Genetic, drug-induced, paraneoplastic
Features of pemphigus foliaceus
Superficial, scaly crust, “corn flakes”, no systemic symptoms
Pathology of PF
Ab against desmoglein 1
Tx of PF
Steroids, mycophenolate mofetil, dapsone, antimalarials
Features of IgA bullous dermatosis
Tense bullae in rosette or string of pearls; adult - drug induced (vanco), peds = chronic form
Pathology of IgA bullous dermatosis
Subepidermal bulla with neutrophils, linear IgA at DEJ
Features of dermatitis herpetiformis
Pruritic vesicles on extensor surfaces; associated with autoimmunity and sensitivity to gluten
Pathology of DH
Subepidermal vesicles with neutrophils at DEF, DIF reveals granular IgA at dermal papillae
Treatment of DH
Dapsone and gluten free diet
