What is Atherosclerosis?
A narrowing and hardening of arteries due to ahterosclerotic plaque.
What vessels are most affected by atherosclerosis?
- Abdominal aorta
- Coronary arteries
- Popliteal arteries
- Carotid arteries
- Vessels of the circle of willis
What are the contests of Plaque?
- Fibrous cap
- Lipid Core (debris, cholesterol, foam cells, calcium)
- Media
What are the steps to Atherosclerosis formation?
- Chronic Endothelial injury
- Endothelial dysfunction with Monocyte adhesion and emigration.
- Macrophage Activation. Smooth muscle recruitment.
- Leukocyte/smooth muscle engulf lipid
- Smooth muscle proliferation and complicated plaque
What are some causes of chronic endothelial injury?
- Hypertension
- hyperlipidemia
- Smoking
- Toxins
- Viruses
- immune reactions
When does cholesterol really become a problem in terms of plaque development?
It has to become oxidized first.
What matters when it comes to cholesterol levels?
The ratio between TG/HDL
What are important factors in the control of ROS?
SOD converts to H2O2, and catalase or Glutathione convert it to water.
What is the rate limiting step enzyme in the production of Cholesterol?
HMG CoA Reductase.
How does Insulin and Glucagon affect HMG CoA Reductase? What do these hormones also do to absorption of cholesterol?
Insulin activates in=increase in cholesterol and absorption.
Glucagon inactivates, decreases cholesterol and absorption
What do statins do? Side effects?
Inhibit HMG CoA Reductase.
Can increase risk of diabetes, Muscle pain.
How do statins cause muscle pain?
Statins inhibit cholesterol production, which is used to create Ubiquinone. This is used in the electron transport chain.
What are the steps of plaque formation?
- LDL-B enters through endothelium
- LDL-B is oxidized
- Oxidized LDL-B attracts macrophages
- Macrophages become lipid laden and turn into foam cells.
- Foam cells accumulate and induce inflammation
- Plaque consists of foam cells and white blood cells
What blood pressure is considered hypertension?
140/90 or above.
