how Barbiturates work

A

allosterically bind the GABA-A receptor and lengthen the duration of opening of the Cl channel causing inhibition in the CNS (Cab-A, now open longer poster)

Barbiturates side effects

A

phenobarbitol: inducer of CYP-450 (car bumper)
cardiac and respiratory depression, hypotension, CNS depression and coma (collapsed heart and lungs on old lady who fainted with brain hair dryer)
avoid in elderly
tolerance and addiction (all are welcome, addicted to curls)

A

long so hangover effects are common(long tapering flag)

Barbiturates uses

A

IV thiopental: induction of anesthesia (the ol quick shave over intubated guy)
IV phenobarbitol: seizure management (construction worker shaking by IV pole)
Primidone: seizures and essential tremor with propanolol (guy with tremor combing the hair of the lady with the perm)

thiopental onset and duration

A

rapid onset, short duration because highly lipid soluble, it is redistributed into skeletal muscle and adipose tissue (the ol quick shave, cut hair redistributing onto belly and arms)
