Autoimmune Disease Flashcards
Autoimmune Disease
- F»_space; M
- middle-age
- positive family history
- multiple co-existent autoimmune diseases
* *Raynaud disease
* *Sjogren syndrome - multiple auto-antibodies
- respond to immunosuppresive therapy
Raynaud Disease/Phenomenon
- W > M
- recurrent vasospasm of fingers and toes
- response to stress/cold
- tissue: white–>blue–>red
- primary–only Raynaud
- Secondary–Raynaud and another AI disorder
Consequences of Raynaud Disease
- tissue may die from repeated vascular injury
- very painful
- wear gloves all the time
Systemic Lupus
- F > M
- butterfly rash uncommon
- photosensitivity rash common (sensitive to sun)
- renal involvement with hypertension
- cardiac lesions (Libman-Sacks endocarditis)
Systemic Lupus-Dental Aspects
- mucosal lesions are indistinguishable from lichen planus
2. slit-like ulcers at gingival margin?
Drug-Induced Lupus
- resembles systemic lupus erythematosus
2. resolves when drug is discontinued
Discoid Lupus
- mucotaneous disease
- indistinguishable from systemic lupus
- serologic abnormalities minimal or absent
- no serious systemic effects
- rare progression to systemic lupus
Discoid Lupus-Clinical Features
- facial/sun-exposed skin rash
- scalp alopecia
- mucosal lesions
- respond to corticosteroids
Discoid Lupus - Dental Aspects
- mimic lichen planus
- biopsy essential
- malignant potential ?
Sjogren Syndrome
- F»_space; M (5% of women; 0.5% of men)
2. middle-age
Primary Sjogren Syndrome
- dry mouth
2. dry eyes
Secondary Sjogren Syndrome
- dry mouth
- dry eyes
- Rheumatoid Arthritis or other CT DZ
What increased risk do people with Sjogren Syndrome have?
- lymphoma (MALT lymphoma)
2. usually parotid
Sjogren Syndrome - Diagnosis
- Serology
- Schirmer test (lacrimation)
- minor salivary gland biopsy (immune destruction)
- salivary flow rate/production
Sjogren Syndrome - Dental Aspects
- mucosal discomfort
- candidiasis
- parotid enlargement
- dysphagia, dysphonia, dysgeusiz
- difficulty wearing dentures
- symptomatic tx
- preventive care
Scleroderma
- hard skin
- localized - morphea
- linerar - coup de sabre
- systemic
* *limited
* *diffuse
* *sine
Graulomatosis with Polyangiitis
1, AKA Wegener granulomatosis
- giant cell vasculitis w/ necrotizing respiratory granulomas
* *nasopharyngeal inflammation
* *pulmonary cavitation
* *renal disease - erythematous, swollen gingiva with strawberry-like texture
- mucosal ulceration
- delayed extraction socket healing
Scleroderma - Dental Aspects
- 80% with head and neck symptoms
- 30% are initial signs and symptoms
- mask-like face (Mona Lisa)
- restricted facial movements
- constricted mouth opening (fish mouth)
- limited tongue mobility (chicken tongue)
- jaw resorption
Urticarial plaques forming blisters, oral lesions
Linear IgA Disease
