What are fertility assessments of the female?

A

Age ( greater than 35)
Duration ( more than 1 yr coitus without contraceptives
Medical hx ( abnormal fat distribution, hair growth- which is indicative of an endocrine disorder
Surgical hx ( pelvic and abdominal procedures)
Obstetric hx ( past episodes of spontaneous abortions)
Gynecologic hx ( abnormal uterine contours or any hx of disorders that may contribute to the formation of scar tissue that causes blockage of sperm or ovum)
Sexual hx ( intercourse frequency, # of partners across the life span and STI’s)
Weight ( over or under weight)
Occupational or environmental exposure

  1. Semen analysis/ In 40% of couples infertility is due to male infertility
  2. Pelvic exam/ assesses 4 uterine or vaginal anomalies
  3. Hormone analysis/ evaluate hypothalamic- pituitary- ovarian axis to include sperm prolactin, FSH, LH, estradiol, progesterone and thyroid hormones.
  4. Endometrial biopsy/ evaluates response, secretory, and luteal phAse of menstrual cycle.

What are 5 infertility tests?

A

Post coital tests/ evaluates coital techniques and mucus secretions
Ultrasonography/ a transvaginal or abdominal ultrasound performed to visualize female reproductive organs
Hysterosalpinography/ outpatient radiogrhoc procedure I which dye is used to assess patency of Fallopian tubes/ assess 4 hx of allergies to iodine and seafood
Hysteroscpoy/ radiograpic procedure where the uterus is examined for signs of defect, distortion or scar tissue
Laparoscopy/ arose dire where gas insufflation under general anesthesia is used to observe internal organs.

What is infertility defined as?

A

The inability to conceive despite engaging in unprotected sex for a prolonged period of time or at least 12 months.

Common factors are; decreased sperm production, endometriosis, ovulation disorders, and tubal occlusions!

What are complications of an ectopic pregnancy?

A

The ovum may implant in the Fallopian tubes or abdominal cavity due to the presence of endometrial tissue.
As the ovum increases in size, the Fallopian tube may rupture, and extensive bleeding occurs, resulting in surgical removal of the damaged tube.
If ectopic pregnancy is identified prior to rupture of tube; surgical removal of the products of conception may be performed, or methotrexate ( Rheumatrex) is prescribed to dissolve the pregnancy.
There is an increased risk of recurrence of ectopic pregnancy a and infertility.
