How does aspirin work?
Inhibits COX
What does COX do?
Its necessary to produce thromboxane A2
Side effects of aspirin
- bleeding
- GI ulceration, bronchospasm (because it blocks production of prostaglandins)
How does clopidogrel work?
ADP receptor antagonist
How does prasugrel work?
ADP receptor antagonist
How does dipyridamole work?
Phosphodiesterase inhibitor - reduces production of cAMP which is a ‘second messenger’ in platelet activation
How does abciximab work?
GP IIb/IIa inhibitors
How do platelets attach to each other?
GP IIb, IIIa and fibrinogen
Which granules stimulate platelet activation?
- thrombin
- thromboxane A2
- ADP
When should you stop anti-platelets if you’re going to undergo an operation?
7 days prior to operation
If you’re on anti-platelets and you have a serious bleed, how can you reverse this?
With platelet transfusion
This causes organ ischaemia and is an inflammatory disorder instigated by damage to the endothelium
