1
Q

(EXAM) Parkland formula for how to ressuscitate a patient with burns

A
  • IV Ringer’s lactate= 4 cc x weight in kg x % of burned body surface area (you put 70 if 70%, not 0.7)
  • give this IV NS in 24 hours but half must be given in first 8 hours

Q

(EXAM) when use Parkland formula, with what fluid do you ressuscitate

A

Ringer’s lactate bc NS is hyperchloremic (more Cl than in our body) and so Ringer’s lactate is more isotonic

Q

(EXAM) how to estimate burn surface % of TBSA

A
  • palm of the hand = 1% = use as reference
  • genitalia = 1%
  • rule of nines: head = 9%, thorax = 18%, back and butt = 18%, each arm = 9%, each leg = 18%

Q

(EXAM) guidelines as to when to send patients to burn center (9)

A
  • age 11 to 49 with 2nd or 3rd degree TBSA>20%
  • age <10 or >50 with 2nd or 3rd degree TBSA >10%
  • 3rd degree burn TBSA>5%
  • 2nd or 3rd degree burn to face, hands or genitalia
  • electrical or chemical burn
  • circumferential burn
  • burn in pt with systemic condition so prolonged recovery and complicated management
  • severe burn with trauma
  • hospitals with children burn pt that don’t have proper personnel or equipment
