Approach To Chronic Leg Ulcers Flashcards
What’s the most common type of chronic lower limb ulcer?
Venous
Commonest cause of venous ulcers?
CVI (chronic venous insufficiency)
Explain the Fibrin Cuff theory of venous ulcers
Fibrinogen forms a cuff around capillaries, leads to interference with oxygen change and hence ischemia
Where are venous ulcers common?
Gaiter region
Where are ischemic ulcers common?
Toes
Conservative Treatment of ulcers
Leg elevation, compression stocking/bandage
Minimally invasive treatment of ulcers?
Injection foam scleropathy/ SEPS/ endovenous ablation.
Where are arterial ulcers common?
Feet, head of the 1st and 5th metatarsals, and lateral aspect of the leg
Compare arterial and venous site?
Venous is generally gaiter region, arterial is sites of trauma or pressure areas, like the heel, feet, so on.
A leg ulcer is chronic after how long?
4-6 weeks.
What ulcer type is commonly associated with punched-out edges?
Arterial ulcers
What sign is specific to malignant ulcers in relation to their edges?
Everted and rolled edges
What dressing type promotes autolytic debridement in ulcers?
Hydrocolloids
