What is the mechanism of action of anti platelet drugs?
Inhibits ADP pathway by binding to PSY12 receptors on platelet cell membranes
What is the importance of the ADP pathway?
It is needed for platelet activation and subsequent platelet aggregation
What is the route of delivery of anti-platelet drugs?
PO
What are the indications for antiplatelet drugs?
- Primary and secondary prevention of cardioembolic events following ACS
- Prior to coronary stenting procedures
- To be taken for 1 year following coronary procedures
How is clopidogrel prescribed for primary prevention of cardioembolic events?
300mg prescribed with aspirin
What anti platelet drug is usually prescribed for secondary prevention of cardioembolic events?
Ticagrelor (with aspirin)
What are the contraindications to anti platelet drugs?
- Active bleeding
- History of intracranial haemorrhage
When should anti platelet drugs be used with caution?
Hepatic and renal impairment (avoid if severe)
Can anti platelet drugs be used during pregnancy and breastfeeding?
Avoid
What might anti platelet drugs interact with?
- Other anti platelet drugs, anticoagulants, NSAIDs, fibrinolytics, SSRIs
- Ulcer healing drugs
- Grapefruit juice

Reduced anti-platelet effects

Decreased efficacy of clopidogrel

- Monitor patients for side-effects during review appointments
- Monitor patients renal function 1 month after starting ticagrelor
What are the common side effects of anti-platelet drugs?
- Abdo pain
- Bleeding
