Antilipemic Drugs Flashcards
What are statin drugs used to treat?
Dyslipedemia; they help lower cholesterol and LDL levels
Describe how statins work
There is an enzyme in the liver that is needed to make cholesterol. Statins inhibit this enzyme which reduces cholesterol production
What are contraindications of statins?
Pregnancy and breastfeeding
Liver disorder (alcohol abuse or high liver enzyme levels)
What drug interactions are there with statins?
-Grapefruit juice
- Warfarin
-Fibrates
-CYP3A4 Substrates (Macrolide antibiotics, azole antifungals, quinidine, verapamil, diltiazem, amlodipine)
For which statin should you avoid food while taking?
Simvastatin
For which statin do you have to take with food?
Lovastatin IR
When should a patient take Lovastatin ER?
At bedtime
When should a patient take Fluvastatin IR?
At bedtime
Name the therapeutically dose equivalents of the statins?
Flu 80
Lov 40
Pra 40
Sim 20
Ator 10
Ros 5
Pit 2
What are the potential side effects for Statin Drugs?
Musculoskeletal: Muscle cramps & myalgias
Dermatological: Rash & pruritis
Neurological: Headache
GI: Nausea, constipation, diarrhea, ab pain, farting, liver dysfunction
What can you do while administering a statin drug to minimize the GI effects?
Administer with a glass of water or a meal
What are the adverse of effects of Statin Drugs?
-Myopathies
-Rhabdomyolysis
-High liver enzyme levels
What is Rhabdomyolysis?
A severe type of myopathy where there is a breakdown of muscle. Leads to high CK levels and myoglobins in the urine. Can lead to acute renal failure and be fatal
What Statin side and adverse effects should be reported to the provider?
-Muscle aches and pains
-Darkening of urine
-Nausea, Vomiting
When should Lovastatin be taken?
In the evening with a meal
Which statins are significantly metabolized by CYP3A4?
-Lovastatin
-Simvastatin
-Atorvastatin
How do Bile Acid Sequestrant Drugs work?
They bind to bile acids which prevents the cholesterol from being reabsorbed by the small intestine. Then it is excreted in the feces
Name 3 Bile Acid Sequestrants
Colesevelam
Cholestyramine
Colestipol
How do Fibrate Drugs work?
They stimulate a receptor that makes lipoprotein lipase which is an enzyme that breaks down choelsterol
Name 3 Fibrate Drugs
-Fenofibrate
-Fenofibric Acid
-Gemfibrozil
Should Gemfiborzil be given before or after meals? Why?
30 minutes before; food reduces the body’s exposure to the drug so it wont work as well
What are contraindications for Bile Acid Sequestrants?
-Biliary & Bowel obstruction
-Pregnancy
What are contraindications for Fibrates?
-Preexisting gallbladder disease
-Liver & kidney dysfunction
-Breastfeeding
Why are statins best given at night?
The liver produces most cholesterol at night
What medication class can cause urine to have a burnt odor?
Bile Acid Sequestrants (Colestipol & Cholestyramine)
Can Ezetimibe be used during prengnancy?
Hell no
Which antihyperlipidemic drug can be used with patients with organ transplantation or severe liver/kidney disease?
Colesevelam
