Antiarrhythmics Flashcards
Class 1 antiarrhythmic
Has local anesthetic activity
Acts on phase 0 of cardiac action potential
Singh-Vaughan Williams classification
Class 1 sodium channel blockers
Class 2 beta blockers
Class 3 potassium channel blockers
Class 4 calcium channel blockers
Class 1a
Prolongs AP duration
Class 1b
Shortens AP duration
Group 1c
no effect on AP
MOA group 1
Block conduction in ischemic and depolarized cells.
Slows down or abolishes abnormal pacemakers
Most selective class 1 group to ischemic tissue
Class 1b
Use dependent for class 1
Selective for tissues that are frequently depolarizing.
State dependent class1 antiarrhythmic
Selective for tissues that are relatively depolarized during rest
Class 1a drugs
Procainamide, disopyramide, quinidine
I Am the Quin who PROClaimed DISO’s pyramide
Procainamide
Blocks Ina and some Ik channels.
Slowed conduction velocity and pacemaker activity.
Post MI arrhythmia
WOF lupus like syndrome
Disopyramide
Marked antimuscarinic effects.
Atropine like effects
Quinidine
Can be used in malaria.
WOF cinchonism - headache, vertigo, tinnitus
Class 1b antiarrhythmic drugs
I Buy MEXIcan TOCAs from LIly
Mexiletine, tocainide, lidocaine
Ib selective for
Ischemic or depolarized Purkinje and ventricular tissue.
