Anesthesia Flashcards

1
Q

Anesthetics must be …… soluble to cross BBB

A

Lipid

Also helpful to have low solubility in the blood so they don’t get stuck there when its time for them to work or its time to get them out.

9
Q

How does GABA work

A

binds GABA receptor, induces chloride channel to open. When chloride channel opens, the postsynaptic membrane is stabilized and it becomes extremely hard for another action potential to occur

Q

So in anethsisa you want amnesia, analgesia, and paralysis

A

The benzos can give you the amnesia what gives you complete paralysis……Neuromuscular blockers

17
Q

Two categories of NM blockers

A

Depolarizing and Non

THe one depolarizer is : Succinycholine. Ach agonist that produces sustained depolarization. Muscle wil contract initially and then block any other Ach binding. You will see fasciculations first.

The Non- depolarizers work by competing with Ach for binding sites. Can be over come by giving Neostigmine, and Achesterase inhibitor that leads to increase in Ach which can overcome the receptor blockade.

18
Q

Neostigmine works how

A

Achesterase inhibitor…inc Ach

19
Q

Phase I block of Succinylcholine

A

No antidote

20
Q

Phase II block of succinylcholine

A

Can be antagonized by neostigmine

21
Q

Succinylcholine Toxicity

A

MALIGNANT HYPERTHERMIA

22
Q

How do you treat malignant hyperthermia

A

Cool , sedate, GIVE DANTROLENE

23
Q

How does Dantrolene work

A

Prevents sarcoplasmic reticulum from releasing calcium

24
Q

Local anesthetics don’t work well where

A

acidic tissue (infected) so you give more

25
Q

Order of events with local anesthetic

A

Small myelinated first

Lose pain, temp, touch, pressure in order

26
Q

Give what with local anesthetic

A

vasoconstrictor (epinephrine) to prevent systematization

27
Q

IV most commonly used for conscious sedation

A

Medazolam

28
Q

Anesthetic with high triglyceride content

A

Propofol
