Indications for use of Amiodarone
Treatment of defibrillation-refractory VF/Pulseless VT, polymorphic VT, and wide complex tachycardia
Rate control in atrial fibrillation or flutter
Mechanism of action for Amiodarone
Effects on sodium, potassium, and calcium channels
Prolongs action potential, refractory period
Ventricular automaticity
Slows membrane depolarization
Slows down heart rate
Dilates coronary arteries
Contraindications of Amiodarone
Bradycardia 2nd or 3rd degree heart block Cardiogenic shock Hypotension Pulmonary congestion Thyroid disease
Adverse reactions of Amiodarone in CV
Bradycardia, hypotension, asystole/cardiac arrest, atrio-ventricular block
Adverse reactions of Amiodarone in torsades de Pointes
(Prolongs QT interval) congestive heart failure
Adverse reactions of Amiodarone in GI & hepatic
Nausea, vomiting,
Adverse reaction of Amiodarone in skin
Slate-blue pigmentation
Adverse reaction of Amiodarone
Fever, headache, dizziness, flushing, abnormal salivation, photophobia
Administration of Amiodarone
IV/IO bolus, IV/IO infusion
Adult dosage of Amiodarone for VF/Pulseless VT
300mg push over 30-60 seconds
May repeat in 3-5 min with 150
Adult dosage of Amiodarone for wide-complex tachycardia, atrial flutter and Fib, SVT with cardioversion
150 mg IV over 10 min
May repeat every 10 min
